The prescribed forms can be obtained at the Admission Office, Site II, Delta State University, Abraka on payment of a non-refundable fee of Five Thousand (N5,000) Naira.

2. Inter-Faculty Transfer – N150000
3. Intra Faculty Transfer – N50000
