3. Go to Admission Office for clearance and issuance of
4. Registration Pin Number
   Log on to fuk.safsrms.com for payment of Registration fees using the pin number
5. Go to Bursary Department for University stamp on your Payment Receipt
6. Fill the Online Course Registration Form

5. JAMB Original Admission Letter (Institution Copy) (coloured)
   Acceptance Letter
   O’ level Statement of Result (s) (browsed copy)/Certificate
   Scratch card for the ‘O’ level verification
6. A’ level Statement of Result/Certificate (DE Candidates)
7. Certificate of Primary Education
   Certificate of Origin/lndigene
8. Certificate of Birth/Declaration of Age
9. Two (2) Passport Photograph
